Swissair Flight 111 is one of the most tragic accidents in the history of aviation involving a McDonnell Douglas MD-11. The accident that happened on September 2, 1998, involved a Swissair McDonnell Douglas MD-11 heading to Cointrin International Airport in Geneva, Switzerland. It was from F. Kennedy International Airport in Newyork, United States. The accident that occurred in the Atlantic Ocean southwest of Halifax International Airport led to the death of all 229 persons on board.

Reliability describes the probability in which a facility or equipment will function correctly and safely over a defined design life under the normal working conditions without failure. A high-reliability organization or business is one that successfully has avoided natural hazards in a company or facility or environment that is prone to accidents due to risk factors and complexity. Air crash investigations have identified that at the time of occurrence of the Swissair Flight 111 crash, a reliability program was in place at SR Technics (Griffioen, 2009). It was meant to ensure airworthiness for the flight as well as a high level of reliability to optimize economic functions. The program existing under the umbrellas of a joint reliability program referred to as KSSU due to its operators like Swissair, SAS, KLM, Union de Transport Aeriens and Air France.

Reliability failures are the stress dependent failures that do not get detected under design operating quality control functions. Equipment with this type of failure successfully pass the initial inspection, often operate well and exhibit standard specification requirements at room temperature. The impacts of reliable failures are detrimental. For instance, the Swissair accident led to the large loss of lives as well as incurred enormous economic losses on the company. Reliability of a product gets tested and retested to increase consistency.
